Change: "An identifier for the UserTokenPolicy assigned by the Server."

To: "An identifier for the UserTokenPolicy provided by the Server, which may be NULL."

Need 1.04 Errata as well.

Added the following clarification:

The identifier may be null or empty but the identifier shall be unique across the UserTokenPolicies assigned by the Server.

Agreed to 1.04 Errata.

This requires further clarification: As null and empty strings are identical, this would mean there cannot be a
null and an empty PolicyId at the same time. Also for a null PolicyId an empty PolicyId would need to be accepted
and vice versa.

Added in UserTokenPolicy.PolicyId:
Null or empty are equal.

Added following sentence to all PolicyId descriptions in all UserIdentityToken types:
Servers that provide a null or empty PolicyId shall accept null or empty as equal.
